Registration for Fall Sale 2008

VERY IMPORTANT TO ALL CONSIGNORS PLEASE READ THROUGHLY! In light of recent Toy Recalls, please be advised of the following: AFUMC is committed to selling only merchandise that is safe for our children. Please verify that all toys and games to be consigned have not been recalled by the manufacturer or the Consumer Product Safety Commission. As a consignor you are responsible for verifying recalls with the Mattel, Fisher Price and other toy manufacturer websites.

Instructions for Online Registration

• Complete the entire form below, make sure you click the submit button at the end
• Consignor Check-in will be handled on a scheduled basis so we can process your items as efficiently as possible, please make sure to sign up for a designated drop off time
• Once all your information is received your consignor number, packet, and pre-made tags will be emailed to you.
